Overview
This article explains how to create a Risk Assessment Template within Risk 365.
Creating a template allows you to tailor assessments to the specific risk areas you want to evaluate. When building a template, you can choose between Manual fields and System fields, giving you flexibility while maintaining consistency.

System Fields
System fields automatically pull information from the REG database and require no input from the assessor.
Manual Fields
Manual fields must be completed by the individual conducting the assessment. These fields allow assessors to apply professional judgement where required.
How to create a risk assessment template
- Open the main menu, select Risk 365, and then choose Templates to open the Templates page.
- Click the New template (
) button.
- Enter a name for your template.
- Select a Scoring type.
- Note: Average scoring requires additional configuration. See the Configuring average scoring guide for details.
- Click Add assessment field (
) button to open the side panel.
- Enter a field name.
- Choose a field type: Manual or System.
- If you selected a Manual field, choose a data type.
- If you selected a System field, choose a Data area and Source field.
- Enable automatic scoring if required.
- Note: Automatic scoring requires additional configuration. See the Enabling automatic scoring guide for details.
- Click the Add field (
) button to save the field.
- Repeat steps 6 –11 to add additional fields to your template.
- Once you have added more than one field, you can re‑order them by dragging and dropping within the table. This determines how they will appear in the Risk Assessment.
- Click the Publish (
) button to make the template available for use across your member for all risk assessments.
- Confirm you would like to publish the template by clicking the Publish template(
)button in the confirmation dialog.

Important
Once published, the template becomes available for use in live risk assessments. Ensure all fields and scoring configurations are reviewed before publishing.
